Kinder mit EB werden als \u201eSchmetterlingskinder\u201c bezeichnet, da die Haut so zart und zerbrechlich wie die Fl\u00fcgel eines Schmetterlings ist. Betroffen sind von der Krankheit nur ungef\u00e4hr zwei bis drei von 100.000 Menschen.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Die Ursache ist erblich bedingt, bzw. sind die Gene ver\u00e4ndert, die den Aufbau von Eiwei\u00dfmolek\u00fclen bestimmen, die wiederum daf\u00fcr sorgen, dass die unterschiedlichen Hautschichten zusammenhalten. Unterschieden wird die Epidermolysis bullosa in die folgenden Formen:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Epidermolysis bullosa simplex<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Dystrophische Epidermolysis bullosa<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Junktionale Epidermolysis bullosa<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Kindler-Syndrom<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Der Unterschied zwischen den einzelnen Formen liegt in der jeweiligen Genver\u00e4nderung und den betroffenen Eiwei\u00dfmolek\u00fclen. Zus\u00e4tzlich zu den zuvor genannten Formen existieren noch rund 40 weitere Unterformen, die sich in Bezug auf die Auspr\u00e4gung der Krankheit und der Prognose unterscheiden. Gemeinsam haben alle Patienten, dass sie eine besonders empfindliche Haut haben.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In der Regel zeigen sich die ersten Symptome wie Blasen, offene Hautstellen und Wunden schon nach der Geburt.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Eine kausale Therapie oder eine Heilung der Krankheit existiert derzeit nicht. Deshalb stehen im Vordergrund die Schmerztherapie, die Wundversorgung der betroffenen Hautstellen sowie die Vorbeugung von Wunden. Sollte es zu Kontrakturen oder Verwachsungen kommen, k\u00f6nnen diese mithilfe eines chirurgischen Eingriffes behandelt werden.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">Das Endocannabinoidsystem und die Haut<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Studien konnten bereits zeigen, dass sich in der Haut Endocannabinoid-Signalwege befinden, die f\u00fcr unterschiedliche Prozesse bedeutend sind. So fanden Forscher heraus, dass der Gehalt von den Endocannabinoiden Anandamid und 2-AG in der Haut nach einem irritierenden Reiz stark ansteigt (1). Vermutlich ist dies eine adaptive Antwort des K\u00f6rpers, um Entz\u00fcndungen und\/oder Schmerzen zu verringern.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In den Hautzellen selbst scheint das Endocannabinoidsystem ebenfalls eine wichtige Rolle zu spielen. Sowohl Anandamid als auch 2-AG k\u00f6nnen die Entwicklung der Hautzellen beeinflussen. Das gilt vor allem f\u00fcr die \u00e4u\u00dferste Zellschicht der Haut (Epidermis), da hier auch Cannabinoidrezeptoren nachgewiesen werden konnten (2).<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">Behandlung mit CBD-\u00d6l<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Die Forscher der Stanford University (USA) pr\u00e4sentierten im Jahr 2018 die Ergebnisse ihrer Beobachtungsstudie an drei F\u00e4llen (3):<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Fall 1: Einsechs Monate altes Baby mit schwerer EB wurde mit einer rezeptfreien Petrolatum-Salbe (Vaseline) und Emu-\u00d6l zur Wundversorgung behandelt. Zudem erhielt das Baby das topische Antibiotikum Mupirocin, das Antihistaminikum Diphenydramin und Morphin. Im Rahmen der Studie wurde regelm\u00e4\u00dfig ein CBD-Spray auf die betroffenen Hautstellen aufgetragen.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Fall 2: Ein dreij\u00e4hriges M\u00e4dchen mit schwer ausgepr\u00e4gter EB wurde mit Kokosnuss\u00f6l, einer Petrolatum-Salbe, einer Zinkoxid Allantoin 6 Prozent Creme und dem topischen Antibiotikum Bacitracin behandelt. Im Anschluss wurde regelm\u00e4\u00dfig eine Mischung aus CBD-\u00d6l und Emu-\u00d6l auf die wunden Stellen aufgetragen.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Fall 3: Ein zehnj\u00e4hriger Junge mit schwerer EB erhielt ein Antibiotikum, das Schmerzmittel Naproxen und das Antiepileptikum Gabapentin, das auch bei Nervenschmerzen eingesetzt wird. Regelm\u00e4\u00dfig wurde dann CBD-\u00d6l auf die betroffenen Hautstellen aufgetragen.<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Im ersten Fall verringerte sich die Blasenbildung und die Wunden heilten schneller. Au\u00dferdem ben\u00f6tigte das Baby weniger Morphin. Auch im zweiten und dritten Fall bildeten sich weniger Blasen und es kam zu einer schnelleren Wundheilung. Zudem war eine deutliche Schmerzlinderung zu verzeichnen. Der zehnj\u00e4hrige Junge konnte sogar die Medikamente Naproxen und Gabapentin absetzen.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">Behandlung mit THC und CBD<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Im Jahr 2019 f\u00fchrten Forscher der University of Groningen (Niederlande) aus, dass die Opioid-basierte Therapie bei der Behandlung der Epidermolysis bullosa unterst\u00fctzend eingesetzt werden kann. Bei einem signifikanten Anteil der Patienten k\u00f6nne jedoch damit keine ausreichende Schmerzlinderung erreicht werden. Da Cannabinoid-basierte Medikamente zunehmend in der Therapie von chronischen Schmerzen untersucht werden, stellen sie eine neue Option in der Schmerzbehandlung bei EB dar.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Die Forscher stellten drei Patienten mit EB vor, denen sublingual Cannabinoid-basierte Medikamente in pharmazeutischer Qualit\u00e4t, bestehend aus Tetrahydrocannabinol und Cannabidiol, verschrieben wurde.
